PATIENT FORMS

Patient forms are required prior to consultation and can either be downloaded and printed from our site or handed out by our receptionist upon entering our office. Consultation and History

A brief explanation of chiropractic will be presented to you at this time. During the consultation you may address the doctor with any questions or concerns that you may have. If the doctor and you feel that chiropractic care is a good fit then a detailed history of your condition will be taken.

Examination

After your history a chiropractic examination will be performed. An examination consisting of orthopedic test, neurological test, muscle testing, reflexes, postural analysis, and x-rays when indicated will be taken.

X-Rays and Special Study Imagery

Imagery may be warranted for your condition in order to specify and assist in developing the most effective treatment plan for you.

Report of Findings

Once all the information taken from your history and the examination is finished, a complete report will be presented to you about your condition as well as the treatment plan. If you have a condition that requires care with other health care providers, the doctor will tell you.

Same Day Treatment

Treatment will take place on the first visit proceeding the detailed history and examination. This may include spinal adjustments, myofascial therapy, stretching, and physiotherapy.

Your Next Appointment

Depending on the condition you will typically schedule a follow up appointment 1-2 days after your first visit. Homework or at home therapy will be given to the patient and will be expected to be perform to help prognosis.
